Student Vaccine Verification

Grays Harbor College is a fully vaccinated campus beginning fall quarter, aligning with the Governor’s July 30, 2021 Higher Education.

Student Attestation Form

All students who participate in on-campus courses, activities, or other operations at Grays Harbor College locations will be required to be fully vaccinated against COVID-19 subject to any legal medical or religious exemptions. The attestation form has been updated to include an option for students who are only taking online courses and not coming to campus for any reason.  

Students have until, Monday, October 4, to verify their vaccination status or indicate whether you will be seeking a medical or religious exemption.  Students that do not submit an attestation by this date will have a hold placed on their account that will restrict their ability to register for subsequent quarters.  

On Friday, October 15, students that have not effectively started this process by this date will be removed from in-person classes and not allowed on campus until their vaccination status has been verified.  

Verify Your Vaccination Status 

  1. By Monday, October 4 verify your current vaccination status by submitting a student attestation form and providing your dosage dates or indicating whether you will be seeking a medical or religious exemption. 

  1. If you are seeking an exemption, download and complete the medical or religious accommodation forms (see below for more information).  

  1. By Nov. 5, 2021 or as soon as possible, email exemption documentation (Medical Accommodation Form or Religious Accommodation Form) to Students will be contacted within 1-2 weeks.

Exemption Information 

When verifying vaccination status in the student attestation form, students may indicate an exemption for medical or religious reasons. Those requesting an exemption must email exemption documentation (Medical Accommodation Form or Religious Accommodation Form) to Students will be contacted within 1-2 weeks. 


By Nov. 5, 2021, in order to grant a reasonable accommodation to a student to remain unvaccinated, Grays Harbor College must receive documentation from a health care provider. That documentation, the Medical Accommodation Form, must confirm that the student is medically unable to receive an authorized COVID-19 vaccine. The documentation must also include a duration the accommodation will be needed. For the purposes of this student medical accommodation request, a Health Care Provider includes all qualified and licensed MD, ND, DO, ARNP, or PA professionals. 


By Nov. 5, 2021, in order to grant a reasonable accommodation to a student to remain unvaccinated, Grays Harbor College must receive information or documentation, the Religious Accommodation Form, that confirms that the student has a sincerely held religious belief that prevents them from receiving the COVID-19 vaccine. The religious exemption must be based on a sincerely held religious belief system that is comprehensive in nature, including both theistic beliefs and non-theistic moral or ethical beliefs as to what is right and wrong which are sincerely held with the strength of traditional religious views. Social, political, or economic philosophies, as well as mere personal preferences, are not religious beliefs; however, overlap between a religious and political view does not place it outside the scope of the exemption. 

Note: Students taking online classes who wish to come to campus must complete the vaccination verification process.
