Veteran's Educational Benefits

The following must be submitted (all at the same time) for the certification process to begin.

Note: Many of the forms specified below seem to have been replaced by an online application tool. If you need to find the actual form, try searching at the U.S. Department of Veterans Affairs: Benefit Forms and/or Education and Training and/or VA Forms.

Applicants Under Chapters 30, 34, & 35 

  • New applicants complete VA Form 22-1990 or Apply Online
  • Transfer students must complete VA Form 22-1195 or Apply Online
  • Submit copies of:
    • DD-214 Report of separation
    • Marriage certificate (upon request)
    • Birth certificates of dependant children (upon request)
    • Divorce decrees (upon request) 

Applicants Under Chapter 35 

  • New applicants complete VA Form 22-5490 or Apply Online
  • Transfer students complete VA Form 22-5495 or Apply Online
  • Submit copies of marriage/birth certificates.

Applicants Under chapter 1606 (Reserves) 

  • New applicants complete VA Form 22-1990 or Apply Online
  • Transfer students complete VA Form 22-1995 or Apply Online
  • Submit a copy of DD-2384, Notice of Basic eligibility (NOBE)
    • Signed by the approving Officer of your unit
    • This form is only valid for 120 days after issue!

Applicants Under Chapter 31 (Vocational Rehabilitation) 

  • This program is a vocational rehabilitation/reemployment program for disabled veterans. Students are pre-approved for this program and will have a vocational counselor assigned to them. They will have a 28-1905 Form, which shows the approved program for the student.
